Our cards can be helpful in situations where communicating or advocating for yourself is difficult.
Our cards are bespoke and can include information specific to individuals.
The cards are made of plastic and are the size of a bank card.
If you are an AIM member and would like an Alert Card, please complete the form below including name, address, date of birth, specific information you would like including on the card, and attach a recent photo of yourself.
Below are a couple of examples of alert cards.
We can help you with the wording if you wish. Please just ask.

South Tyneside AIM members can request a Health Passport.
The passport is supported by staff at South Tyneside Emergency Department.
Health passports can be presented upon arrival to support autistic people in accessing healthcare.
Passports provide communication support and the sharing of important information which may not otherwise be known in an emergent circumstance.
Passports support staff to reasonably adjust interactions. They may also be used in other health care settings.
Please contact us if you live in South Tyneside and would like a passport.
