Do you live in Sunderland or South Tyneside and have a diagnosis of autism or are on the waiting list for diagnosis?
We’re offering free, personalised Reasonable Adjustment Cards to help you feel more confident and understood when you're out and about.
These small, discreet cards can explain your communication needs or how others can support you – they are perfect for showing in healthcare settings, shops, public transport, or any situation where you might need a bit of understanding.
You choose what the card says – we make it just for you.
We are currently offering these through local GP surgeries and via our website.
What is it? A Reasonable Adjustment Card can help autistic people to communicate important information about their needs in health settings, such as sensory sensitivities, communication preferences, or any reasonable adjustments that can make appointments more accessible.
The cards can contain a photograph, personal details, and an emergency contact number as well as detailing reasonable adjustments.
The cards are credit card sized to fit easily into a purse, wallet or a phone case.
Presenting the card can help healthcare professionals and other organisation/services better understand how to support the holder and ensure their experience is less stressful and more tailored to their needs.
Who can apply? Sunderland or South Tyneside residents or people registered at a GP practice in Sunderland or South Tyneside, who have an autism diagnosis OR are currently on the waiting list for an autism assessment.
Why it matters: Autistic people often face barriers in healthcare and other settings - from noisy waiting areas to rushed appointments. This card empowers them to ask for adjustments that make services more accessible and less stressful. For example: A quiet waiting room, extra time for appointments, clear written information, support with communication etc.
If you need support to apply for a reasonable adjustment card, our Health Link worker can:
Support you to identify the reasonable adjustments you may need
Help you to complete the application form/complete the form on your behalf
Ensure the completed Autism Alert Card is delivered to a GP surgery, posted directly to you, or be available to collect from one of our health hubs in Pennywell or Washington.

The front of the card details personal information and a photograph.
The rear of the card contains bespoke information about the reasonable adjustments required by the card holder.
